IRB Roster

Purpose:  The document describes how the UW-Madison maintains its IRB membership lists (rosters) and the content of those rosters.

Policy

I.      The UW-Madison will maintain lists of each IRB’s members that include the following information with respect to each member, including each alternate member:

A.  name
B.  earned degrees
C.  representative capacity
D.  indications of experience (such as board certifications, subject area expertise, title and/or department, if UW-Madison employee)
E.  affiliated or non-affiliated status of each member, including employment relationship with UW-Madison
F.  whether the member is a primary member or alternate member
G. if an alternate member, the primary member or members for whom the alternate may serve as substitute
H. whether the member is a scientific or nonscientific member

II.     Alternate IRB members

A.  Alternate members of each IRB serve as substitutes for those primary members designated on the IRB roster.   When serving on a convened IRB as substitute for a designated primary IRB member, an alternate has full voting rights and all the responsibilities of a primary member.  An alternate may also serve as primary reviewer and preliminary reviewer with respect to protocols.
B.  Alternate members of the IRBs receive the same membership training as primary members.  
C.  IRB minutes will identify when an alternate member is present at a convened meeting and for which primary member they are substituting.

III.    Lists of primary and alternate members shall be updated each year upon reappointment as an IRB member.

IV.   Any changes to IRB membership will be reported to the Office of Human Research Protections (OHRP) in a timely fashion by the IRBs through the UW-Madison Office of Research Policy.
