Policy: We require the Principal Investigator signature on all forms. In addition, the Principal Investigator must also sign all correspondence submitted to the Health Sciences IRB Office. This includes notification of adverse events, response to modification notices, etc. Please include the following statement: "I have read the above and take full responsibility for it." This should be followed by the Principal Investigator's signature and the signature date.
